• Cosmetic Dentistry Included

Membership Benefits:

• Initial comprehensive exam or routine exam, all necessary x-rays and 2 prophylaxis  within 12 months of enrollment
• All x-rays within 12 months of enrollment
• All dental treatment completed in house within 12 months of enrollment will be discounted by 10% if paid in full at time of service
• This includes elective cosmetic procedures like fillings, crowns, dentures, and veneers.

“The Fine print”

This is a dental discount plan and is NOT dental insurance. It CANNOT be combined with any other dental insurance. It is good ONLY at Sierra Family Dental. Therefore a specialist, will NOT offer this discount. Should there be dental treatment needed following any type of injury where a lawsuit and therefore outside medical, car, disability or workman’s comp type insurances are involved, this discounted plan cannot be used.
